We ask that all grant applications be submitted through our online application system and include the following information:
- Name, address and telephone number of the organization and appropriate contact person
- Agency history (including mission)
- Description of program activities and goals
- Board of Directors/Trustees listing
- Total cost of the project
- Amount being sought from the Mutual of Omaha Foundation
- Target population and number of people served by the program for which the grant is being requested
- Budgets for the agency and program
- A list of other sources of support and the amounts and types of committed and pending donations
- An explanation of whether this is an on-going activity or a one-time project
- Inclusion of other groups with which this effort is being coordinated
- Measurement method by which the success or failure of the project will be gauged
Note: The Mutual of Omaha Foundation may, at a later time during the review process, request a copy of the organization’s most recent audited financial statement.
Within two weeks of submitting a grant request, applicants will be notified of the date their request will be reviewed. Communication regarding the outcome of the grant review will be provided within seven days following the review.
